The diagnosis is one of exclusion in which thyroiditis and tumor and the possibility of endemic goiter are ruled out. It is thought that the goiter represents a com¬pensatory respOnse to an impaired efficiency of the thyroid gland. Hypersecretion of TSH may lead to stimulation of glandular growth and morphologic changes. The thyroid gland may be diffusely enlarged and smooth, or it may be grossly nodular. In the early stages of development, the gland is hyperplastic.
